Applicant must use the current, official Rotary Scholarship Application Form. It must be dated and signed by the student.
Beginning Freshman must furnish an official high school transcript of the student's grades from the beginning of the 9th grade through the final semester of 12th grade. (May be photocopies that bear an original signature of the proper school designee.) Students already enrolled in a college or technical program must furnish an official transcript showing all courses taken and grades received from all colleges or institutions of higher learning.
If you need more room to list your activities and offices, honors and awards or church and community service, you may include an additional page. Be sure to label your additional sheet the same as the application form.
The applicant must prepare a typed statement of 300 words or less setting forth his/her vocational or professional goal and how past, present and future activities and scholarship make the accomplishment of the goal probable. This statement must be signed and dated by the applicant.
Copy of parent’s or legal guardian’s most recent Income Tax Return OR W-2’s are required unless the applicant has reached the age of 24. Applicant must then provide copy of his/her Income Tax Return, W-2’s or Student Financial Aid Report (FAFSA).
